Using information gathered through the intake process, a customized Pathways Plan is developed to identify strengths, growth priorities, recommendations, and meaningful next steps.

Collaborative Planning Meeting
Once the Pathways Plan is drafted, we meet together to review recommendations, answer questions, refine priorities, and ensure the plan reflects your young adult’s goals and family priorities.
